Procedures

1) Natural decease in the workplace of an employee who was registered and listed in the Health scheme:


a) The seafarer’s family members are required to send Fanimar a letter through registered mail notifying the decease that took place on land after disembarkation due to illness. They are also required to attach the following documents, necessary for the Steering Committee to validate the claim:
•Name Surname / Date of birth / Nationality / Residence and phone number of seafarer / family members
•Notification of landing due to illness, with clear and detailed reason for the landing.
•First medical report by the Ministry of Health and certificates attesting duration of illness up to decease.
•Certification attesting to the nature of the illness that caused the decease.
• Shipowners’ Company Declaration (with copy of evidential document) attesting to the contract type (R.O./C.R.L./ T.P./T.G./Foreign Flag) existing on the date of the accident between the said Company and the employee.

b) Following the Steering Committee’s deliberation and upon written demand by Fanimar, the seafarer’s family must submit the following documents to complete the filing procedures:
• Certificate of Death (original copy)
•Self-certification of statutory declaration (original) attesting to the heirs’ identity
•Written authorization (with original copy of statutory declaration) to collect the entire sum in favour of one heir, on behalf of all heirs.
•Authorization to receive compensation (original) by an appointed judge if there are minors of age among the heirs
•Bank account information if a money transfer is preferred.

2) Death due to natural causes of an employee who was registered ‘de facto’ following Union Agreements and under the terms of said agreements, having taken place on land and unquestionably due to the illness that led the seafarer to land:

3) Industrial accident in the workplace - that is on board - entailing permanent impairment exceeding 5% (certified and paid by one of Fanimar’s Insurers), to an employer who is registered in the Fund due to union agreements and under the terms of said agreements (National Union Agreement of 20.12.2002 and following amendments):
The applying seafarer will have to wait for the Steering Committee’s decision to allocate the benefit, which consists of a 25% integration of the sum that was previously paid by the insurer. No document whatsoever will be needed aside from the one sent to claim compensation.


4) Permanent revocation of Navigation Document due to illness or casualty to a Commander or Chief Engineer who has automatically joined the fund following Union Agreements and under the terms of said agreements, after visit by the 1st grade medical commission and by Fanimar’s Insurers’ fiduciary doctor:
The applying seafarer will have to wait for the Steering Committee’s decision to allocate the benefit, which consists of an integration amounting to the same sum that was previously paid by the insurer. No document whatsoever will be needed aside from the one sent to claim compensation.
